1. 간질
1) 단독요법 : 6세 이상의 소아 및 성인에서의 2차성 전신발작을 동반하거나 동반하지 않은 부분발작 치료
2) 부가요법 : 기존 1차 항전간제 투여로 적절하게 조절이 되지 않는 2세 이상의 소아 및 성인에게서 다음 질환에 사용된다.
- 2차성 전신발작을 동반하거나 동반하지 않은 부분발작
- 레녹스-가스토 증후군과 관련된 발작
- 1차성 강직성/간대성 전신발작
2. 편두통의 예방
2) 용법,용량

이 약은 낮은 용량에서 시작하여 유효용량까지 증량하는 것이 바람직하다.
이 약의 투여를 최적화하기 위해 혈중농도를 모니터할 필요는 없다.이 약의 권장용량은 신장질환이 없는 경우, 노인을 포함한 모든 성인 및 소아에게 그대로 적용한다.
식사와 관계없이 투여할 수 있다.
정제는 부수지 말고 그대로 복용해야 한다.
1. 간질
1) 단독요법 : 단독요법으로 토피라메이트를 투여하기 위해 병용중인 다른 항전간약물을 중단할 경우, 발작조절에 미칠 수 있는 영향을 고려해야 한다. 안전성 문제로 다른 항전간약물을 즉시 중단해야 하는 경우를 제외하고는 2주 간격으로 다른 항전간약물을 1/3정도로 단계적으로 감량 중단하도록 한다(사용상 주의사항 "5. 일반적 주의"항 참조).
약물대사 효소 유도 약물을 중단시, 토피라메이트 농도가 상승할 수 있다. 만약 임상적으로 토피라메이트의 농도 상승이 나타날 경우 이 약의 용량을 감량할 필요가 있다.
성인 (17세 이상) :
처음 1주일 동안 저녁에 25mg씩 투여를 시작한다. 이후, 1-2주 간격으로 1일 25 또는 50㎎씩 증량하여 1일 2회로 나누어 투여하도록 하며, 만약 환자가 이러한 증량 경과를 견디기 어려워지면, 증량간격을 길게하도록 한다. 용량이나 증량속도는 임상적 반응에 따라 결정한다. 성인에 대한 단독요법으로서의 권장용량은 1일 100～200mg이다. 최대 권장용량은 1일 500mg이다. 재발된 간질 환자 중 일부는 토피라메이트 단독요법시 1일 1,000㎎까지 내약성이 있었다.
소아(6-16세)
처음 1주일 동안은 저녁에 0.5-1mg/kg으로 투여를 시작한다. 이후 1-2주 간격으로 1일 0.5-1㎎/㎏씩 증량하여 1일 2회 투여하도록 하며, 만약 환자가 증량을 견디기 어려워하면, 증량단위를 줄이거나, 증량간격을 늘리도록 한다. 용량이나 증량정도는 임상반응에 따라 결정한다.
소아에 대한 단독요법으로서의 권장용량은 1일 3-6mg/kg이다. 부분발작으로 진단된 소아의 경우 1일 500㎎까지 투여한 경험이 있다.
2) 부가요법
성인(17세 이상)
최소유효량은 1일 200mg이며 통상 1일 200-400mg을 2회로 나누어 투여한다. 환자에 따라 최대 권장용량인 1일 800mg까지 투여할 수 있다. 처음 1주일 동안은 저녁에 25-50mg씩 투여하는 것으로 치료를 시작하며, 더 낮은 용량에서 시작하는 것도 가능하다. 이후에는 1-2주 간격으로 용량은 25-50mg씩 증량하여 1일 2회로 나누어 투여한다. 만약 환자가 증가용량을 견디기 어렵다면 더 적은 용량을 증가시키거나 증량 간격을 연장시킬 수 있다. 투여량은 임상효과에 따라 결정하여야 한다.
소아(2세-16세)
1일 권장용량은 약 5-9mg/kg이며, 1일 2회로 나누어 투여한다. 처음 1주일 동안은 25mg(또는 그 이하, 1일 1-3mg/kg을 기준으로 함)을 저녁에 투여하고, 이후에는 적절한 임상효과가 나타날 때까지 1-2주 간격으로 1일 1-3mg/kg씩 증량하여 1일 2회로 나누어 투여한다. 용량 적정은 임상효과에 따라 결정하여야 한다. 1일 30mg/kg까지 투여된 경우도 있으며, 이 경우 일반적으로 내약성이 좋았다.
2. 편두통의 예방
성인 : 처음 1주일 동안은 저녁에 이 약 25mg으로 투여를 시작한다. 이후 1주 간격으로 25mg/일 씩 증량한다. 만약 환자가 이러한 증량을 견디기 어려워하면, 증량간격을 길게 하도록 한다. 1일 권장용량은 100mg으로 1일 2회로 분할하여 투여하도록 한다. 일부 환자에서는 1일 50mg 용량에서도 효과를 나타낼수 있다. 1일 200mg까지 투여할수 있으며, 용량 및 증량 속도는 임상반응에 따라 결정 하도록 한다.
3. 신장애 환자
- 중등도 및 중증 (CLcr < 70 mL/min/1.73 m2)의 신장애 환자는 시작용량 및 유지용량의 약 1/2로 감량하는 것이 권장된다.
- 혈액투석환자: 이 약은 혈액투석으로 혈장에서 제거되기 때문에 혈액투석환자는 투석 당일에 이 약 투여용량을 약 1/2 증량한다. 투석에 걸리는 시간, 투석장치의 청소율 및 투석환자의 신청소율 등을 고려하여 투여량을 증량하고, 혈액투석의 시작과 종료 시에 분할 투여한다.
4. 간장애 환자
간장애 환자들은 이 약의 클리어런스가 감소될 수 있으므로 주의하여 투여하여야 한다.

For the love of a dog!

Okay, first - this toy is a hoot. When you first get it and hear it's cute grunting/tooting sound, you can't help but grin and think, Yup! That sounds like somebody in here just tooted! Which for some reason always does seem to be a bit funny, doesn't it? And you'll probably find yourself chuckling a little and saying, Oh how cute. After that, the next logical step is you'll introduce the toy to your dog. And then, depending on your dog's particular personality, you may soon discover (as we did) the amazing love/hate relationship a human can develop with a simple dog toy.
We have a 1-yr-old standard poodle whom we named Kenda. And yes, he is named after Joe (for any of you ID fans out there). His official AKC registered name is Lieutenant Kenda, Home Inside Hunter. Corny? No doubt. But it truly seemed an appropriate name for him, because this is the first dog we've ever owned that actually LOVES to play with dog toys and will endlessly hunt them down throughout the house. No toy, however well hidden, stands a chance with this determined toy hunter. As Joe might say, he WILL find you! :) His toys are his friends, and he is fiercely devoted to them.
Enter the adorable little rubber hedgehog with his even more adorable "toot". The moment Kenda laid eyes (or ears?) on this little guy, all other toys were forgotten. It was love at first sight. So much so that within a few hours of him playing with this toy to the exclusion of all others, we decided to give him a name. We call him "Blue" (I know, we're so creative). Blue immediately became Kenda's best friend - or at least his best toy. He played with him constantly. He bit him, he wrestled with him, he chewed on him. He brought Blue to us and, if we were sitting down, very carefully placed this slobbery ball of rubber in our laps, as if asking, Can we play catch with Blue? Huh? Pretty please?? Sometimes we did, sometimes we didn't. On those occasions when we didn't, he would play catch with himself, picking Blue up in his mouth, swinging his head, and tossing him across the kitchen; then running/sliding across the kitchen floor to retrieve him on the other side of the room. In the beginning, if Blue was nowhere in sight (and with dogs, out of sight is usually out of mind), the hubby and I would get a kick out of saying, "Kenda, where's Blue?!" Just for the enormous fun of watching a 55-pound poodle suddenly leap a foot in the air, scramble his legs mid-air like Fred Flintstone getting his car started, then half running/half sliding across the kitchen hardwood floor in a desperate effort to find his beloved Blue. Oh, how we entertained ourselves in those early days watching Kenda with his Blue. And through it all, through every bite, squeeze, toss, push, throw, and chew of this toy.......the toot. The grunt. Okay, let's call it what it really sounds like, folks: a FART, okay? There, I've said it. It sounds like your grandpa just passed gas - bigtime. Funny? At first, yes. Hilarious. But a thousand times a day? Over and over and over? While you're trying to talk on the phone? While you're trying to have conversation with each other over coffee at the end of the day? Sometimes for an hour NON-STOP? Well, let's just say the humor of it all began to elude us a bit. And therein lies our love/hate relationship with this adorable little toy.
We thought we'd died and gone to heaven one day when Blue stopped tooting. Turns out Kenda had chewed on him so much his tooter (located rather anatomically correctly in his tushie) had fallen out. Or rather IN, since it was now in Blue's tummy. Poor Blue, he couldn't make noise anymore, and although Kenda kept playing with him you could tell he was confused as to why his little buddy had fallen silent and wouldn't "talk" to him anymore. And as much as the hubby and I were enjoying the tooting reprieve, we couldn't take it. By the third silent day, I could almost feel the invisible hands of Amazon coaxing me toward my computer, gently urging me to buy another Blue. But I resisted, folks. I did NOT buy another Blue. I bought TWO more Blues! One for now, and one for that possible future day when this Blue, too, falls silent. Why? Because ... well, because it's BLUE! He's practically a member of the family now. The dog loves Blue, and we love the dog. I guess it's that simple.
My final word on this dog toy? It's adorable. It's well made and will hold up to a ton of play and chewing. His tooter may not survive as long; I guess that remains to be seen. And if your dog is anything like mine, well then your sanity may take a hit as well. But if your dog loves his little hedgehog buddy as ours does, and if you love your dog (and you know you do!), then you might decide your sanity is worth the risk.
Two thumbs way, WAY up!
P.S. Blue now has a friend. We just bought the pink sheep. Kenda is in 7th heaven. Our house sounds like a retirement home after a chili bean supper. And yes, we named him "Pink". I told you - we are nothing if not creative.

Strong hold

Very good product. I’m definitely impressed. I have two German shepherds and this toy is definitely durable and does what it’s made to do. The suction is great. The company even gives you a clear plastic strip that you can actually put on the floor and then put the suction cup on it For better grip. I’ve had suction toys before which was absolutely horrible. This one definitely supersedes all others. The suction comes off once in a while but usually that’s because if my dogs are literally biting on the suction cup part lol. But with confidence. Great product

Base of you is very durable
Color: Blue
Overall this is a fun toy. My German Shepherd loves it. The ropes were chewed off within the first hour which was disappointing. Using it for treats did not work because the opening is so large. It has become only a chew toy and it is very durable. Since I bought this to enjoy the extra features, I am disappointed. I was able to buy three chew only toys for the same price. I still give it four stars because my dog likes it so much.

The dog will destroy the ropes and eat them be careful with this

The rubber part is very durable but the strings or ropes are not. Puppy is eating the strings and having a hard time pooping them out. Just cut them off close to the knots and watch for them to eat the strings they pull off.
